  To my brilliant medico mates, Dr Stephen Barnett and fellow writer Dr Katie Pullinger, thank you for straightening out the details relating to illness and death. Any remaining errors are mine, and probably due to the fact that most of what you said went straight over my head. Grant Tucker and Karen Abey – how lucky that my family have friends!
