Chapter 720 - Widowmaker Heart Attack

Chief Jiang once again confirmed that Zheng Ren was not joking. He walked to the operating table in confusion and looked at Zheng Ren. He asked, "Doctor Zheng? What's wrong?" "Have you ever had a heart attack before?" "Yes but that was a long time ago. There's nothing abnormal now." "Lie down. Slowly. Don't use too much force." Zheng Ren's words were filled with authority and could not be rejected. Su Yun looked at Zheng Ren in surprise. He couldn't have really diagnosed Chief Jiang as having an acute myocardial infarction, right? Chief Jiang was stunned. He did not spend much time with Zheng Ren but Zheng Ren had long conquered him with his unparalleled surgical techniques. What he was saying was all true... He laid on the operating table in a daze and smiled. "Doctor Zheng, you should seize the time to rest for a few hours." Chief Jiang thought that Zheng Ren's behavior was abnormal because he was tired. He should not anger him now. The two of them had completely different thoughts. Things had become a little strange. After Chief Jiang lay down, Su Yun pushed the electrocardiogram machine over. He skillfully lifted Chief Jiang's shirt and stuck the pads to Chief Jiang's c.h.e.s.t. It was a little inconvenient for Zheng Ren to look at the electrocardiogram so he was not in the mood to do it. Therefore, Su Yun was doing it. It was impossible for a person who could do a heart transplant to not be able to see the electrocardiogram well. "Boss, acute myocardial infarction in the anterior wall is recommended. He requires emergency surgery." Su Yun looked at the electrocardiogram, and his expression immediately became cold and indifferent. He gave a cold and accurate opinion as if he was treating any normal patient. "Where is Chief Jiang's wife?" Zheng Ren asked the junior doctor. "Uh..." Chief Jiang grabbed the electrocardiogram. The characteristic manifestation of myocardial infarction was clearly shown on it—ST-segment elevation. "That's impossible." Chief Jiang was puzzled. If it was true, why didn't he feel anything at all? Zheng Ren glanced at it and said, "The ST-segment elevation from V1 to V6 shows that it's an acute thrombus. The thrombus exists at the proximal end of the left anterior descending artery. Other than the anterior wall being affected, the ventricular septum and the lateral wall will also be affected. "You know what? Call Chief Jiang's wife and tell her to sign for the emergency surgery," Zheng Ren said. "Prepare the emergency surgery and medicine." The air in the operating theater froze for a few seconds but no one moved. Zheng Ren looked at Chief Jiang in confusion and asked, "Do you usually perform resuscitation like this?" Chief Jiang's old face turned red. Although he still did not believe Zheng Ren's words, he was now the patient so how could the emergency surgery not be performed after the Doctor's orders? Just as he was about to speak, Zheng Ren said calmly, "This is a typical widowmaker heart attack. According to the official definition of STEMI given by the American College of Cardiology and the American Heart Association's guidelines, you're in a critical stage of the widowmaker heart attack so you must undergo emergency surgery without delay." Chief Jiang recalled the electrocardiogram. Although he still did not believe it, the objective evidence was there and he could not refute it. "Boss, do you want to remove or dissolve the thrombus?" "Remove the thrombus," Zheng Ren said. "Do you have a catheter to remove the thrombus from the coronary artery?" Chief Jiang put himself in the position of a doctor. If he were to encounter such a situation, he would definitely need emergency surgery. However, it was very difficult to remove the thrombus from the left anterior descending artery! That was still not the main point. The main point was that he was a doctor. How could he lie on the operating table during emergency treatment and delay their resuscitation efforts?! "Don't be stubborn. If we hurry up with the surgery, we can save some trouble. If we argue with you again, you might not be the only one who can't be saved. The others might not be saved either," Su Yun immediately retorted when he saw Chief Jiang's constipated expression. Chief Jiang felt as if cold water had been poured over his head. He instantly became clear-headed. "Prepare for surgery. Xiao Liu, bring me an A4 paper," Chief Jiang said in a deep voice. The junior doctor was stunned by the scene in front of him. It was not until Chief Jiang spoke that he went to fetch the paper in a daze. Chief Jiang signed his name on the paper and said, "Go and sign it before the surgery. I'm not considered a wounded soldier at the frontline. I'm considered an ordinary patient in the emergency department." In his mind, the emergency department chief's heart attack made him nothing more than an ordinary patient. "Slowly take off your shirt," Zheng Ren said. Chief Jiang smiled bitterly. As he took off his shirt, he whispered, "If I suddenly had a heart attack and couldn't be saved, would I be considered a martyr?" "No," Su Yun said, "With me... With the boss and I here, it's not that easy for you to die. It's just a widowmaker heart attack. It's not that difficult to treat. Don't dawdle. After ten minutes of surgery, you can rest for the day." "I can finally rest?" Chief Jiang lay on the cold operating table in a daze. "Su Yun, you can accompany me to the operating table and disinfect things," Zheng Ren said. "You're wasting my time!"Su Yun ran to brush his hands. As he did so, he said, "If you're tired, you should just rest rather than actually having a heart attack. That's why my boss is here. If we wait for you to have symptoms, we won't even be able to remove the thrombus." "It can be removed," Zheng Ren said. Speechless, Su Yun glanced at him and said, "Boss, I'm complimenting you. Can't you tell?" "Hurry up and disinfect. There are still injured people over there," Zheng Ren said indifferently, ignoring Su Yun's nagging. The operating theater was emptied. It was like an emergency treatment for the injured. The surgery quickly began. Chief Jiang felt that he had entered a wonderful world. One second ago, he was still rescuing the wounded from the frontline. The next second, he was lying on the operating table, quietly waiting for the operation. Life was really f*cking amazing! He cursed in his heart. Why was he so disappointing?! Why did he have a heart attack when he was fine?! It was a pity that he could not help with resuscitation anymore. Thinking about it, Chief Jiang felt very regretful. His vision was blurry. "Hey, Old Jiang, you're not going to cry, are you?" Su Yun disinfected his left wrist and did not lose the opportunity to retort. "No."Chief Jiang closed his eyes and lied. A stream of tears flowed down the corner of his eyes. "It's a small problem but let me tell you, you have to rest well after the surgery. This is considered a work-related injury." Su Yun said, "And don't make a fuss about supporting the sick. Don't throw yourself into the cause of saving lives and helping the injured. You'll just be a nuisance. You'll be fine. This will make things much better in the future." Chief Jiang did not expect Su Yun to tell him this. The sadness disappeared into thin air. "Simply put, don't come up and cause trouble. I'm just giving you some advice." "Brother Yun, you don't have to comfort people like this, right?" Chief Jiang smiled bitterly. "Comfort? Nonsense! Hurry up so we can be done here. There are still injured people waiting for you." Su Yun nagged but did not delay his actions at all. Zheng Ren finished brushing his hands, put on his clothes, and stood next to Chief Jiang. "Doctor Zheng, thank you for your trouble." "You have a stent, right?" Zheng Ren did not notice Chief Jiang's change of mood at all. He only treated him as an ordinary patient who needed treatment. "Xiao Liu brought it over," Su Yun said. "Yes." Zheng Ren began to lay out the sheet and an artery sheath appeared in his hand.

  Chapter 721 - Was Hot-Blooded

Chief Jiang had performed hundreds and thousands of circulatory interventional surgery and had seen many more surgeries. Some of them were done on his end, some were while wearing lead clothes and watching from behind teachers, and some were while sitting beside the operating room and watching on a screen.

However, this was the first time Chief Jiang had seen such a view from the operating table.

On the upper left field of his vision was the screen. The image on the screen was the angiogram of the last patient who had been embolized. He knew that the area would soon start moving. What appeared was the condition of his coronary artery.

Whether it was an anterior myocardial infarction or not, he would know with just a glance.

  No
t only that, he suddenly realized that he could observe Zheng Ren's surgery from the patient's perspective.

Just as he thought of this, Chief Jiang suddenly realized something—would Doctor Zheng be doing interventional surgery that was usually done by the circulatory surgery department?

As this thought arose in his mind, the mind that had just calmed down suddenly became nervous as a sharp pain came from the anterior region of his heart.

"Expand the crown!" Zheng Ren immediately said when he saw that the electrocardiogram displayed on the ECG monitor was beginning to become chaotic.

The nurse at the side table immediately began to inject medicine into Chief Jiang.

"Old Jiang, can't you be more obedient?" Su Yun was a little impatient.

It was mainly because he was familiar with Chief Jiang. Su Yun knew that the more casually he spoke on stage, the more his nervousness would be alleviated. After all, he had never seen Zheng Ren perform interventional surgery on the heart. If he acted as if he was facing a great enemy, the consequences would probably be very serious.

However, he did not expect that he would still have an acute heart attack.

"I... I'm just...wondering if Doctor Zheng will do it." After the medicine was injected, Chief Jiang felt that the pain in the anterior region of his heart became slightly better.

"Lie down. If he can't do it, no one will," Su Yun said disdainfully. "The Nobel Prize judge, Dr. Mehar..."

"Don't talk about that. Once he gets excited, his blood vessels will spasm. If he has another attack, it will be even more troublesome," Zheng Ren said calmly.

"Such a good-for-nothing!" Su Yun glanced at Chief Jiang and reprimanded him.

In just a few words, the local anesthesia at the radial artery of the wrist, the artery sheath, and the catheter had already been inserted.

Zheng Ren began to step on the line, and the contrast agent was injected into Chief Jiang's coronary artery.

Chief Jiang tilted his head to look at the screen and sighed. This speed was as fast as ever. So was he. What was he worried about?

Brother Yun had mentioned the Nobel Prize. Perhaps things would be alright. Chief Jiang stopped pondering on all his tumultuous thoughts and looked at the screen to empty himself.

But before he could empty himself, his thoughts were immediately interrupted by the angiogram on the screen.

The angiogram found that the left anterior descending artery had a limited severe centripetal stenosis. The stenosis was almost at a hundred percent, and the length of the lesion was about 10 to 15mm.

There was no obvious abnormality in the circ.u.mflex artery. The second moderate centripetal stenosis of the right coronary artery was about 50% and the length of the lesion was about 15mm.

The diagnosis was correct. This diagnostic level was really awesome! Chief Jiang was impressed.

Some hints could be given on the electrocardiogram but it was still very difficult to make such an accurate judgment.

"Boss, you said it's been almost two hundred years since Bernard first inserted a catheter into an animal's heart in 1844. I feel that there hasn't been any breakthrough with interventional surgery of the heart." Su Yun said something completely out of nowhere as he looked at Chief Jiang's imaging.

"It shouldn't be calculated like that." Zheng Ren removed the catheter and said, "If we start calculating, I think it was in 1929 when the German doctor Forssmann first inserted a urinary catheter into his elbow vein and sent it through the superior vena cava into the right atrium. We can only say that interventional surgery of the heart truly began then."

As he spoke, Zheng Ren had already sent a 7f JL4.0 catheter through the blood vessel sheath to the opening of the left anterior descending artery. He first sent the 0.014′′ PTCA lead wire to the distal end of the left anterior descending artery.

"Remove the embolic catheter," Zheng Ren said.

The scrub nurse in her own lead apron followed him in and handed him a catheter. Su Yun opened the packaging. "You said that robotic surgery could be carried out within 20 years. Other than not eating the thread, there was no other progress. Using da Vinci robotic surgery for thoracic surgery and general surgery is like making a mockery of all our hard work."

"As long as it can treat the illness. To actually solve these problems from the root cause, we'd have to study biochemistry and genetics." Zheng Ren sent the embolized catheter near Chief Jiang's left anterior descending artery.

Chief Jiang felt that this was what the atmosphere in the operating theater was supposed to be.

The surgeon and the assistant chatted with each other. It was simple and relaxed. It was not like before when everyone was doing surgery in silence and countless patients were waiting for treatment. The pressure was so great that it made people feel like they would collapse at any moment.

All of a sudden, he felt as if his entire life had returned to normal.

However, he was now a patient lying on the operating table. Other than that, everything else was normal.

Oh, there was something else that was not normal. That was Zheng Ren's hand speed. It was simply too fast!

After removing the embolic catheter and entering, he did not seem to think much. In fact, Chief Jiang did not feel anything at all. The embolic catheter on the screen carried a black thing and began to walk out.

Was it completed just like that?

  Chief Jiang was very confused.

If he were to remove this kind of fresh embolic catheter himself... His technical level was not up to par, so he could only allow the catheter to dissolve the embolic catheter.
